FEES & REFERRAL

• Reduced fees are available for Health Care Card, Pension Concession Card or Commonwealth Senior Health Card holders.

• Medicare rebates are available when your GP refers to mindfulBeing Melbourne for Pain Physiotherapy using the Enhanced Primary Care Plan (EPC). Provider details can be found on the referral form below. The Medicare rebate covers part of the cost of up to 5 treatment sessions. Out of pocket fees apply.

• Private Health Insurance: Please check with your private health provider if you have Physiotherapy included in your policy and confirm the specifics of what your policy covers. Receipts will be provided for processing any eligible rebates with your insurance company.

• WorkCover or TAC clients require a referral from your GP or medical specialist with claim details.

• NDIS clients are eligible using a ‘self-managed’ or ‘plan-managed’ funding scheme.
